When a Tooth Should Be Removed
Most teeth can be saved with a filling, a crown, or a root canal. Extraction becomes the right answer when:
- The tooth structure above the gum is too damaged for a crown to anchor
- A vertical root fracture extends below the gum line
- Infection has not resolved after root canal treatment, or has reached the bone
- Advanced gum disease has destroyed the bone holding the tooth, and the tooth is loose
- A wisdom tooth is impacted, infected, or pushing on the neighbors
- A baby tooth is preventing a permanent tooth from coming in normally

Dr. Rainford has been practicing dentistry in Georgia for more than 15 years. He earned his Doctor of Medical Dentistry (DMD) from the Medical College of Georgia School of Dentistry. Before dental school, he completed his undergraduate degree in chemistry at the University of West Georgia. His clinical interests cover the full range of family and cosmetic dentistry, with particular focus on the integration of extraction, bone preservation, and implant restoration.

The Tooth Extraction Process at Our Office
From the first call to your follow-up, here is what to expect:
Before the visit
You call (678) 810-1100 and book a consultation. If you are in active pain, ask about a same-day appointment. New patients without insurance can use the $129 New Patient Promotion to cover the exam, full digital X-rays, and oral cancer screening.
At the consultation
Dr. Rainford reviews your X-rays, your medical history, and the tooth in question. You hear the diagnosis in plain English. If extraction is recommended, you get a clear plan, sedation options, a cost estimate, and answers to every question before the appointment is booked.
On extraction day
You are numbed fully with local anesthesia. Sedation, if you chose it, is started. The tooth is removed gently using the technique appropriate for its position and condition. A bone graft is placed at the same time if an implant is planned for later. Sutures are placed if needed. The whole appointment usually takes 30 to 90 minutes depending on whether the extraction is simple or surgical.
Going home
You leave with written aftercare instructions, gauze for any continued bleeding, and a follow-up plan. If you had oral sedation, someone drives you home.
Comfort and Sedation Options
- Local anesthesiais part of every extraction. The area is fully numb. If you feel anything sharp, we add more numbing.
- Nitrous oxide(laughing gas) is breathed through a small mask. You feel relaxed but stay awake. Effect wears off quickly.
- Oral sedationis a prescription medication taken before the appointment. Stronger relaxation. You stay conscious but remember little of the procedure. Requires a driver.
- The Single Tooth Anesthesia (STA) Systemis used for targeted numbing without numbing a wide area of your face. Useful for extractions where only one tooth needs to be addressed.
Recovery, Aftercare, and What to Watch For
Most simple extractions heal completely in 7 to 10 days at the gum level, with the bone underneath taking several weeks to fully fill in. Surgical extractions add a few days to the active recovery. Your written aftercare instructions cover:
- Bleeding management with gauze pressure
- Swelling control with ice the first 24 hours, warm compresses after
- Pain medication timing
- Food restrictions: soft and cool the first day, no straws, no smoking, no spitting
- Light salt-water rinses starting day two
- Signs to call us about: heavy bleeding past the first day, severe pain that starts 3 to 5 days after (possible dry socket), fever, or a foul taste
Tooth Replacement Planning After Extraction
If the extracted tooth is one you want to replace, the conversation starts before extraction day so the right preparation happens. The options:
- Dental implant: a titanium post placed in the bone that holds a crown. Closest replacement to a natural tooth. Best for visible teeth and load-bearing molars. Requires good bone (or a graft to build up the bone). The FICOI fellowship credentials mentioned above are the implant credentials.
- Bridge: a row of crowns where the middle crown spans the gap. Anchored to the teeth on each side. Works well when those teeth need crowns anyway. Does not require surgery.
- Partial denture: a removable replacement. Less expensive. Useful when multiple teeth in a row are missing or when implants are not an option.
- Full denture or All-on-4: when many teeth are coming out, a full denture or an implant-supported denture (All-on-4) may be the right answer. We see patients for this regularly.
Bone grafting at the time of extraction is often the right move when an implant is planned. The graft fills the empty socket with material that helps your body build new bone. It protects the implant site from collapse. Without a graft, the bone in that spot can shrink, which makes implant placement harder later.

Do I need a root canal or an extraction?
A root canal saves the tooth by cleaning out the infected nerve and sealing the tooth. An extraction removes the tooth entirely. Whether you need one or the other depends on how much healthy tooth structure is left, whether the root is fractured, and the condition of the surrounding bone. Dr. Rainford reviews your X-rays and discusses both options when both are realistic. If a tooth can be saved with a root canal, we tell you. If not, we explain why removal is the safer path.
Can children get tooth extractions at Vibrant Smiles?
Yes. We see patients of all ages including children. Baby teeth that will not come out on their own, teeth being removed for orthodontic reasons, and damaged teeth from sports injuries are common reasons children come in for extractions. We adjust our approach for younger patients and use age-appropriate explanations to keep the visit calm.
What are your office hours?
Monday 9:00 AM to 5:30 PM, Tuesday 8:00 AM to 5:00 PM, Wednesday 10:00 AM to 7:00 PM, and Thursday 9:00 AM to 5:30 PM. The Wednesday evening hours are a good fit for patients who can't take time off work or school. Same-day emergency appointments are available for active tooth pain.
Will I need to replace the extracted tooth?
Often yes. When a tooth is removed, the teeth around it can shift, the opposing tooth can drift, and the jawbone in that spot starts to shrink over time. The main replacement options are dental implants, bridges, and partial dentures. Dr. Rainford's FICOI Fellowship means implant planning happens at our office without needing a referral.
