Surgical Extractions and Complex Cases

A surgical extraction is needed when a tooth cannot be loosened and removed with hand instruments. This includes:

  • Teeth broken off at the gumline with no part visible to grip
  • Teeth with curved, hooked, or fragile roots that would break during a standard extraction
  • Teeth surrounded by dense bone that has fused tightly to the root surface

A surgical extraction may involve a small incision in the gum, removing a small amount of bone around the tooth, and sometimes sectioning the tooth so it can be removed in pieces. The site is closed with dissolvable sutures when needed. Recovery takes a few extra days compared to a simple extraction, but the careful technique reduces trauma to the surrounding bone and gums.

Sedation and Comfort Options

Local anesthesia is included with every extraction. We do not start until the area is fully numb, and we add more anesthetic if you feel anything sharp during the procedure.

For longer procedures or for patients who feel anxious about dental work, two additional options are available:

  • Nitrous oxide (laughing gas): you breathe a mixture of oxygen and nitrous through a small mask. You feel relaxed but stay awake. The effect wears off in minutes, so you can drive yourself home.
  • Oral sedation: you take a prescription medication before the appointment. You stay conscious but feel deeply relaxed and remember little of the procedure. You need someone to drive you home and stay with you for a few hours after.

We talk through which option fits your situation during the consultation. There is no upcharge for being numb fully. There is a separate fee for the sedation options.

What Recovery Looks Like

A simple extraction typically heals within 7 to 10 days for the gum and several weeks for the bone underneath. A surgical extraction (including wisdom teeth) takes a few extra days of more careful aftercare. Common parts of the recovery:

  • Light bleeding and oozing for the first few hours, controlled with gauze pressure
  • Swelling that peaks at 48 to 72 hours, managed with ice on the cheek the first day and a warm compress after
  • Soft food for the first 24 hours: yogurt, smoothies (no straw), applesauce, lukewarm soup
  • Light salt-water rinses starting the second day
  • No spitting, smoking, or vigorous rinsing for the first 24 hours to protect the blood clot

Dry socket is the most common complication. It happens when the blood clot dislodges before the gum has healed over. It is more common with lower wisdom tooth extractions and with patients who smoke. If you have severe pain that starts 3 to 5 days after the extraction, call us. Dry socket treats easily but needs to be looked at.
